Overview

Incident Reporting and Investigation course overview

Treating an incident investigation as a way to assign blame, rather than understanding the underlying causes, does little to prevent it from happening again. This one-day Incident Reporting and Investigation course provides a practical introduction to understanding incidents, identifying contributing factors, and supporting effective investigation processes in line with the Health and Safety at Work Act 2015 (HSWA).
 
We don't do ‘death by PowerPoint’ or overly theoretical training. Instead, our industry-experienced trainers get learners stuck in, using examples and scenario-based group work that reflect the reality of New Zealand workplaces. Participants build practical skills in reporting and investigating incidents, understanding barriers to reporting, and applying structured approaches such as multiple causation and The Defences Model to identify underlying causes.

Participants leave with a clear understanding of reporting requirements and the confidence to contribute to effective incident investigations and support improved health and safety outcomes in their workplaces.

Our approach

We teach this course through facilitated discussion and scenario-based activity that mirror the kinds of incidents people deal with in day-to-day operations. Learners work through how events unfold, what information needs to be captured early, and how gaps in reporting can affect both the investigation and the quality of the final report.

The course focuses on helping learners understand how multiple causation, root cause thinking, and The Defences Model apply during workplace investigations. Participants work through contributing factors, reporting decisions, and evidence gathering in a way that keeps the learning connected to real operational situations and the practical demands of incident investigation.

This course is delivered over one day.
 
Training is available in person nationwide or live online via Zoom for in-house delivery, and is offered publicly as part of the HSR Stage 2: Risk Assessment and Incident Investigation course.
